TO INITIATE MEDIATION SERVICES through the Department of Defense (DoD) Roster, organizations must submit specific documentation (using the Roster’s forms) via email to osd.pentagon.ogc.mbx.doha-mediation-requests@mail.mil These are:
1. Request for Mediation
2. Agreement(s) to Mediate in Good Faith signed by all individuals who will be involved in the mediation session, including:
· The employee challenging one or more management employment actions.
· The DoD management official representing the organization.
· In addition, prior to the mediation, an Agreement to Mediate in Good Faith must be signed by any party representatives or other attendees who will be participating in the mediation.
Scheduling of a mediation will not begin until completed forms are received.
